We welcome submissions from families, friends, and caretakers who would like to honor someone lost to a rare disease. Each name submitted will be added to this memorial page with the utmost care and respect.

Anyone who passed away due to, or in connection with, a rare disease.

Immediate family, friends, caregivers, or legal guardians. Please ensure you have permission from close family before submitting. Rare Living Foundation is not responsible for names submitted without family approval.

What you can include:
Full Name of your loved one
Optional: Age at passing, date of birth/passing
Optional: A short tribute (50–150 words)
Optional: A photo (high-resolution preferred)
Optional: Rare disease(s) they were diagnosed with

Submissions can be celebratory, heartfelt, solemn, or simple—whatever feels authentic to you and your loved one’s memory. -
Submissions are reviewed by our team to ensure respectful and appropriate content. Minor editorial changes may be made to correct typos and ensure easy readability. You will be notified once your tribute is published.
